This restaurant offers a mixed experience with some dishes being excellent and others just okay. The service was top-notch, with our waiter being friendly, attentive, and helpful in guiding our menu choices. The cheese plate we ordered was particularly outstanding, featuring a diverse selection of cheeses with different flavors and textures, accompanied by fruits, nuts, jam, and honey. The ceviche was also delicious, with the perfect balance of acidity and crunch from the diced vegetables and fish. Watching the pasta being tossed on the cheese wheel was a fun experience, although not as dramatic as some may expect. I can't believe it took me so long to try this place but it was worth the wait and yes the hype is real!! Best Italian in Santa Monica and def top 10 in LA. Arancini perfect crispy on outside soft on the inside perfect ingredients. Burrata was top notch, beets were a nice touch. Fettuccine Bolognese INSANE perfect flawless. Wowowow. Paired it with a nice bottle of Montpulciano. Waiter was well versed with wine/ food pairings. Not that I needed help, but I tested him a little to see if he was knowledgeable and he certainly was. Ambience was nice a little on the smaller side but cozy. Can't wait to go back!

Forma is considered one of the top restaurants in Los Angeles. The selection of cheeses is amazing and the pasta dishes are even better. A must-try is the "dalla forma" section of the menu, where the pasta is served inside a cheese wheel. I have visited Forma several times, tried a variety of dishes, and have always been impressed. The tortellini is my favorite, but I also adore the risotto. The service is consistently excellent, the wine selection is fantastic, and the ambiance is charming. I highly recommend dining at Forma.

Thought the food was pretty good. Service was a little confused and it took like an hour for each course, which is not bad, just unexpected. Liked my lasagna and chopped salad. The tiramisu was awesome. Nice little spot on Montana. Dietary restrictions: The gluten-free options for bread and vegan options for cheese were available, so it is possible. Great spot for a cozy Italian meal! Fresh mozzarella knots and grilled spanish octopus appetizers were good. Loved the crispy potatoes that came with the chicken mushroom. The Nutella bread pudding was good but thought it should be called Nutella custard instead. There were hardly any pieces of bread in the serving we got. This is a great restaurant if you like to sample cheeses as they have a wide variety!

Everything was perfect. We liked the setting, modern architecture and the atmosphere. Food was perfect ! The Cheese size is small but we could still share it. I'm a picky eater and it's rare that I find multiple items that I like to order from a restaurant. Format is the best! The cheese, the pasta, the salad, the entrees, I can't wait to go back and try more items. Starting the truffle cheese is a must! I love this place!
